Registering with the surgery
To register with The Churchill Clinic:
- Use the online form
- Visit the surgery after 10am to collect a paper registration form.
You will need to come into the practice to sign the forms and provide evidence of your identity and address (if you have them).
Once you have registered, you must book and attend a new patient health check so we have all your information as soon as possible.
Find your NHS number
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Named accountable GP
All our patients have an allocated named GP. Dr Hannah Newens is your registered GP.
Please note that you can still choose to see the GP of your choice.
